Thomas "Tom" Costabile, born in Chicago, IL, on April 19, 1958, passed away in Oaklawn, IL, on September 3, 2023, following his battle with liver failure.

Tom was the cherished son of the late Carmen Phillip Costabile and late Frances (Messina) Costabile. Divorcee of the late Linda Johnson DelGallo. Brother to the late Marietta (Edward), late Victoria (Wayne), Linda (Mike), late Carmen, and late Louie Costabile. Father to Thomas (Natalia) and Derek (Jackie) Costabile. Uncle to Kimberly (Mark) Lawless, Pamela (Dominic) Pontarelli, Billy Conley, Kenny (Mary) Prause, Jennifer (Danny) Flynn, Jeaneen (Tony) Sica, and Anna (Jimmy) Bennedikt. Nephew and cousin to many.
